GSIS Disability Claim Benefits for Covid-19

Government employees who are enrolled to the GSIS (Government Service Insurance System) can avail claim benefits should they contract Covid-19 (Coronavirus disease 2019). Through this process, monetary compensation can be obtained which can significantly help patch the expenses of the Covid survivor since at times, hazard pay may not be given during the duration of the person's quarantine period.

The following requirements must be complied:

1. Swab test result
2. Medical certificate
3. Certificate of employment
4. Service record
5. Job description
6. Attendance / DTR (this will indicate that you did not reported to work during the duration of your quarantine)
7. Dully filled up GSIS Disability Benefits Income Benefits Claim for Payment. You can download this form from the GSIS website https://www.gsis.gov.ph/downloadable-forms/ under DISABILITY section. The form is titled Disability Benefits.

Note:

A positive swab result proves that you were afflicted with Covid-19

You can obtain the medical certificate from where you were check-upped and been diagnosed with Covid-19. Either from your company clinic or CESU.

A certificate of employment proves that you are working in your respective institutions during the span of time you had Covid-19. You can obtain this from the Human Resources department of your company.

Also, do get a service record from the Human Resources of your respective employment. This document contains details about your employment record and past performance.

A copy of job description reflects your duties and responsibilities in your respective institution.

A copy of attendance or DTR shows that you had not reported to work during the span of your quarantine.
